 A co-coder is responsible for  coding/abstracting acute care services under ORC responsibility. This is an entry level position and coding and abstracting job types may include clinical and ED facility professional fees.

 Responsibilities:

•  Reads and interprets documents  in  medical records to identify and code all appropriate diagnoses and procedures using coding software and following state and PHS-ORC coding guidelines, coding clinic and other appropriate coding references and tools to ensure correct coding assignment. 
• Selects key diagnoses and procedures based on knowledge of coding and UHDDS standards, CMS requirements and potential payment systems. Ensures the correct MS DRG is assigned for appropriate hospital reimbursement. Ensures that APR DRG disease severity  and mortality risk  values ​​are accurate for reporting purposes. I
• f conflicting or unclear information is available according to the appropriate PHS procedures, consult the physicians. Accurately assigns a Presentation of Acceptance (POA) Indicator for each coded diagnosis according to CMS requirements as published in the official ICD-CM Coding Guidelines and consults with physician when in doubt. Accurately assigns discharge code with special attention to DRG transfer program to ensure appropriate hospital reimbursement.
• Abstract medical history information according to CMS requirements and PHS intake procedures. Uses available tools to check records for accuracy.
• This may include information about clinical trials and quality control activities.
• Communicate with coding support staff regarding  patient changes, canceled appointments, misindexed reports and missing chart sections  to ensure complete and accurate coding and proper billing to Medicare, insurance companies and patients.  Performs following activities which may include, but not limited to:

 Termination of incomplete coding when requested reports or completed physician surveys become available. Review and respond to QI Review Coordinator recommendations for all assigned letters. Fulfillment of 72-hour/2-hour batch requests at fixed intervals. Keeps abreast of office practices and changes in coding practices and regulatory information. Comply with all applicable federal, state and local laws, regulations and requirements and PHandS policies and procedures in all aspects of work. Make the necessary changes in work tasks immediately. Attends and participates in code department meetings and training. Completes AHIMA authorized continuing education credits and keeps coding information updated as needed. Contributes to the team's efforts to meet coverage, processing and quality standards by being collaborative, responsible and positive. Share new information with other employees in the department.  Promote PHandS as a premier service organization by treating customers with compassion and respect.  Maintain the confidentiality of all patient demographic, medical and financial information by ensuring proper handling and disposal of confidential records and HIPAA compliance.

Required Qualification:

•   Medical Terminology and A&P Completion (Confirmed)
•  Enrollment in an AHIMA accredited health information technology program or online coding program
•  Proven track record of proactively identifying, resolving and escalating issues affecting business operations and documentation 
•  Good decision-making skills based on analysis, experience and judgment
•  Organizational and planning experience demonstrating the ability to effectively manage time and achieve results in a fast-paced environment

 Desired Degrees:

•  Associate Degree in Health Information Technology
•  Completion of an AHIMA-accredited health information technology program or self-study course by passing  the national  RHIA or RHIT certification exam
•  AAPC or AHIMA code credentials to hire
•  1 year of HIM background/experience or equivalent work experience  in healthcare or clerical/office positions
•  An epic experience
