 Required Qualifications

•  An active pharmacy license in the state where you work. Not on the DEA Excluded Party List.
• Immunization certificate through an accredited organization (ie, APhA)* Listed on  pharmacy state license as "Pharmacist-in-Charge." Submission of required information/documents to  state PMP administrator to register for PMP use (in states where PMP is active  for pharmacist use).
