 Required Qualifications

• Active pharmacy license in the state where you work ∙
• Not on the DEA's Exclusion List ∙ Immunization certificate through an accredited organization (ie APhA)* ∙
• Listed on the pharmacy's state license as "Pharmacist in Charge" ∙
• Submission of information/documentation required  to validate PMP to administrator to register for PMP use (in states where PMP is active in the pharmacy).
