Main tasks and desired results:

• Obtain verbal consent and take all necessary steps to obtain written consent.
• Building collaborative relationships with clients (participants/program members), families, doctors and other providers to determine medical history and current status and evaluate options for optimal results.
• Consumption promotion through education and health protection.
• Assess members' health status and treatment plans and identify gaps or barriers in health service delivery. Create a documented patient-centred case management plan that involves all appropriate parties (client, physician, provider, employer, etc.), identifies expected case outcome/outcome, criteria for case closure, and facilitates communication between all parties involved.
• Implement, coordinate, monitor and evaluate the case management plan on an ongoing and appropriate basis.
• If necessary, negotiate the level, intensity, and duration of treatment with the provider.
• Act as a timely and proactive liaison between accounts, clients/families, physicians and facilities/agencies
• Maintain accurate case management intervention (system) records, including cost benefit analysis, savings and data collection.
• Adhere to professional practice in licensing and certification quality assurance standards and all case management policies and procedures
• Comply with all accreditation mandates, state and federal
• May offer utilization review services while members are in active case management
• Participate in unit and company training initiatives and show evidence of continuing education to maintain clinical experience and certification where appropriate.
• Demonstrate sensitivity to culturally different situations, customers and clients.
• Based on experience, can provide leadership, mentor/mentor, support and coverage to other case management staff and assist case managers in achieving positive results and savings
• Involve our CAC partners (Embedded Care Coordinators) to help with our customer service.
• In addition, the candidate will be responsible for embracing and demonstrating the cultural beliefs of Consumer Health Engagement. You are responsible for modeling the six cultural beliefs to drive personal accountability and organizational results.

Qualification:

• An active unlimited Registered Nurse (RN) license is required.
• Eligibility for additional state licenses based on business need as required by Cigna.
• Certification as a case manager is desirable
• A minimum of three years full-time direct clinical care to consumers
• Clinical experience in emergency care is required
• Excellent time management, organization, research, analysis, negotiation, communication (oral and written) and interpersonal skills
• Good intermediate to advanced computer skills are required
• Must have access to high speed broadband cable internet with modem/router. Cigna does not support hotspots or satellite internet or hotspots for these locations
• This role is a WAH that allows work to be done from home. Employees must be fully vaccinated upon arrival on site.
• If you work from home occasionally or permanently, your Internet connection should go through a cable or fiber-optic broadband Internet service provider with a download speed of at least 10 Mbps/5 Mbps upload.
• For this position, we expect an annual salary of $70,400 to $117,300/year depending on relevant factors such as experience and geographic location.
• This feature is expected to also qualify for participation in the annual bonus plan.
